- Entwerfen & Reflexion: Wurden Interventionsmöglichkeiten kooperativ entworfen und mit Szenarien (Best/Worst/Nichtstun) reflektiert?

|
- Feinziele: Sind sie nach SMART formuliert und Grobzielen zugeordnet (BZ/UZ)?
|
|
- W-Fragen: Beantwortet der Interventionsplan für ein Feinziel konkret «Wer macht wann, was, wie, warum?»
|
|
- Kooperation: Sind klient*innenspezifische und inter-/intraprofessionelle Kooperationen konkret dargestellt?
